It is offered to patients who have proven themselves as individuals who take their oral hygiene and general dental health seriously.

Patient Will Receive:

• One set of custom fabricated upper & lower trays with starter kit at NO CHARGE, additional trays available for purchase.
• Maintenance kit of whitening gel at every recall exam appointment at NO CHARGE, additional tubes available for purchase.
• 50% OFF a one hour ZOOM! Whitening procedure, once per year.
• 5% OFF on smile makeovers, such as veneers, once every 5 years.

To Be Eligible for the Program:

• Agree to and sign the whitening consent form.
• Do not have active, untreated disease – gum disease, decay, root sensitivity, oral cancer, etc.
• Have had a complete dental examination with recommended records, such as radiographs(x-rays) and photos within 6 months.
• Are current with keeping your recommended hygiene appointments.
• To receive any promotional offers, your account must be current.
• Must be at least 18 years of age.
• Must comply with minimum required dental care as treatment planned by doctor.*

Lifetime Maintenance Rules:

• Must maintain continued hygiene care.
• Lost or destroyed applicators or trays will be replaced at cost to the patient.

*All patients will receive their whitening package upon completion of all necessary dental treatment as determined by the dentist.  Minimum gum and teeth health required to receive professional whitening in order to prevent complications that may arise with unhealthy teeth and gums.
